According to Getty (where the original image came from), it's a small dog, but it doesn't say exactly what's going on. I'd guess pleural effusion, too, probably with congestive heart failure. Either way, that is some nastiness going on right there.

I'm going to have to take some pictures of the rads from work, some of them (like the ones for our ortho patients) are pretty cool!
